In the case of breast augmentation, the size of the silicone implant depends on the height, width and depth (the profile). The shape of the breast implants

A distinction is made between round and anatomically shaped silicone implants when viewed in profile. In the case of round implants, the implant width corresponds to the implant height (basic type: round; at Polytech “Même”). Three different height dimensions are available for the anatomical implants (base type): extended base (implant higher than wide; with Polytech “Optimam”), round base (implant as high as wide; with Polytech “Replicon”) and shortened base (implant flatter than wide; with Polytech “Opticon”).  

The surface

In our clinic, silicone implants with a smooth and textured surface as well as implants with a polyurethane coating (PU foam) are available for breast augmentation. The surface is so important because it is in contact with your tissue. Here, the surfaces lead to different tissue reactions. In this context, we are often asked what happens when the body rejects the implants. Our answer to this is always the same: your body will always classify the implants as foreign and, as a reaction to the rejection, it will form a very delicate and fine capsule around the implant after the procedure, which you can neither see nor feel. In the case of implants with a smooth surface and those with a textured surface, this capsule forms separately from the implant. The smooth silicone implants do not bond with the capsule, and the textured implants adhere more or less to the capsule from the inside, but can also detach again. In the case of implants with a polyurethane coating, the capsule forms inside the foam. This reduces two risks: capsular fibrosis is less likely to develop and rotation or displacement of the implants to the sides or downwards is extremely rare. They remain in their initial position for months and can only be moved (e.g. pushed) over time.

Filling the breast implants

When filling modern breast implants, a distinction is made between an aqueous saline solution and an adhesive (cohesive), rather firm silicone gel. We do not offer breast augmentation with saline-filled implants. Instead, we use implants with a silicone gel filling, as we believe that silicone implants with a saline filling offer more disadvantages than advantages. We will be happy to explain the reasons for this in a personal consultation. In addition to silicone implants with only one firmness of silicone gel, we use a series of breast implants filled with two different gels, the so-called “4Two implants”, for breast augmentation. These are generally firmer than the implants in the “Sublime” series. The choice of implant is ultimately a question of taste.

The quality of silicone implants

All the silicone gels we use are cut-resistant and therefore cannot leak. We only use intensively tested brand implants for which the manufacturer offers a lifetime guarantee against material defects in the silicone implants. We therefore do not consider it necessary to change the breast implants if there are no complaints, but this can be done at any time for aesthetic reasons.

Access via the underbust fold

The approach via the inframammary fold is our preferred method for inserting breast implants during breast surgery at the TRAUMKLINIK. This approach has several advantages over the other approaches (armpit and areola). Firstly, the scars are barely visible when the implant is inserted via the underbust fold: initially inconspicuous, they fade over time to form very fine lines that are usually concealed by the breast. Secondly, the implant pocket can be prepared most precisely here, which leads to an optimal result. This approach causes little tissue damage, which is why the risk of bleeding is also low. In addition, the risk of infection or capsular fibrosis is lower as the breast implants do not come into contact with bacteria from the armpit or areola. Access via the underbust crease is via a small incision in the natural crease under the breast. This incision creates a space for the implant, which can be placed either under or over the pectoral muscle. The incision is then closed with fine stitches to minimize scarring.

Choice of implant position

The breasts themselves consist of glandular tissue and fatty tissue. The large pectoral muscle lies underneath. The glandular tissue can temporarily increase in size during pregnancy and especially when breastfeeding, but then often shrinks to a smaller volume than before pregnancy (involutional atrophy). Adipose tissue, on the other hand, reacts to diet-related weight fluctuations. If these alternate frequently, the breasts unfortunately often shrink over time. The extent to which breastfeeding as well as weight gain and weight loss affect the volume of the breasts therefore depends on the percentage of the breasts made up of the two tissues. During the consultation, you will be informed about the different approaches and the position of the breast implants, which can be placed under the muscle (submuscular) or between the muscle and the mammary gland (subglandular).

Breast augmentation over the muscle – epipectoral position

If there is sufficient subcutaneous fatty tissue, it is possible to place the breast implants under the glandular body (subglandular). In this case, the silicone implants lie above the large pectoral muscle (pectoralis major). This placement of the implant is called epipectoral or subglandular. However, as the breast implants are rarely completely covered by the glandular tissue, they lie directly under the subcutaneous fatty tissue in these areas and can be visible and palpable if the fatty tissue is very thin or becomes thinner over time. This can be particularly disturbing in the décolleté, especially if edges are visible and lead to an unnatural, “made” appearance. A loss of fatty tissue can occur due to the pressure of the implant from the inside or due to weight loss. The more of your own fat you have before the procedure, the better: this means that the implants are better covered by the fatty tissue after the procedure. During a personal consultation at our clinic, we will advise you in detail as to whether this method of breast augmentation is suitable for you.

Breast augmentation under the muscle – submuscular position

For breast augmentation at our clinic in Böblingen near Stuttgart, there are three possible implant placements under the pectoral muscle (submuscular):
  1. The traditional form in which the pectoral muscle is not cut at the ribs. This placement is very rarely chosen today.
  2. The total position under the muscle, in which the implant is also covered by muscles in the lateral part. This placement is also rare today.
  3. The so-called “dual plane” position, which is what is usually meant today when we talk about “under the muscle” (pectoral muscle). In this form, the muscle part that attaches to the ribs at the bottom is separated and the pocket for the silicone implant is then formed under the muscle. The inserted implant is thus covered by the muscle at the top and in the middle (in the décolleté), but only by a skin-fat mantle at the bottom and outside. The silicone implant therefore lies under two different tissues (planes), the muscle tissue and the skin-fat tissue. This is why the term “dual plane” is used.
If the soft tissue mantle is genetically unsuitable, placement under the pectoral muscle is always better than above the muscle.

What speaks in favor of a submuscular position of the breast implants

At our clinic, we generally recommend a submuscular position for breast augmentation implants. The reasons for this are as follows: The extended soft tissue coverage of the breast implants creates a more harmonious shape and the implant is less visible and palpable. It can also be assumed that the result will remain stable for longer, i.e. the breast will sag less quickly than in the position over the muscle. Finally, the capsule that surrounds the muscle adheres to the non-displaceable ribs at the back and thus ensures greater stability.

What speaks against a submuscular position of the breast implants?

Special anatomical conditions such as a very accentuated underbust crease with a slightly sagging breast and a short distance from the nipple to the underbust crease can lead to an increased risk of a double-bubble if the muscle has to be cut to position the implant under the muscle. Breast augmentation with autologous fat

Sometimes it happens that the breasts on the left and right are not only different sizes, but are also positioned unevenly on the rib cage. They then have different “footprints”, for example when one breast is lower and further out on the ribcage. The areolas and nipples can also be unevenly positioned on the left and right. This can make it look as if the breasts are “looking” in different directions. Unfortunately, these forms of asymmetry cannot be changed by simply enlarging the breasts. If breast augmentation is combined with a breast lift, at least the size and position of the areolas and nipples can be changed. This is particularly useful if the larger breast hangs significantly more than the smaller one, as every augmentation lifts the breasts to a certain extent according to the volume. However, if the firmer, smaller breast were to receive the larger implants and the more sagging, larger breast the smaller ones, the latter would probably still sag after the procedure. General risks associated with surgery are post-operative bleeding, infection, healing disorders and unsightly scarring. Special risks associated with breast augmentation are the rotation of the implants and the formation of capsular fibrosis or BIA-ALCL(breast implant-associated anaplastic large cell lymphoma). Rotation or lowering of the implants

If the pocket for the implant is significantly larger than necessary, rotation of the implant is a not uncommon risk. Capsular fibrosis

Every foreign body in our body forms a delicate sheath, a capsule made of the body’s own connective tissue – including around a breast implant. If this shell becomes thicker than normal due to increased connective tissue formation, this is known as capsular fibrosis or capsular contracture. In this phenomenon, the connective tissue hardens in a spherical shape and deforms the implant palpably and visibly – up to and including a painful deformity. The risk of developing capsular fibrosis depends, among other things, on the choice of implants. The risk is highest in the first year after the procedure. There does not appear to be a clear cause for the formation of capsular fibrosis – it is rather assumed that there are various factors that promote or prevent the formation. For example, breast implants with a rough surface and the implant position under the pectoral muscle are considered to be factors that make capsular fibrosis formation more difficult. A pectoral muscle with a good blood supply could also contribute to a lower capsular contracture rate due to the protection against bacteria and the constant movement of the muscle. Capsular fibrosis is divided into different degrees of severity:
  • Grade I – size and shape are natural and soft
  • Grade II – size and shape are natural and a little firm
  • Grade III – Size and shape are abnormal and firm
  • Grade IV – size and shape are abnormal, hard and painful to the touch
Treatment is necessary for grade III and IV. The capsule can be incised in several places to relieve the shape-changing tension. This procedure is called a capsulotomy. If the capsule has to be (partially) removed, this is called a capsulectomy. If no new silicone implants are inserted, a breast lift is often necessary to shape the breast. After capsular fibrosis, breast implants with a polyurethane shell are often used to reduce the risk of recurrence of capsular fibrosis.

Breast Implant Illness/BREAST IMPLANT ILLNESS (BII)

The term “breast implant illness” or “BREAST IMPLANT ILLNESS” (BII) is appearing more and more frequently in the media and social networks and is causing uncertainty among many patients who are interested in breast augmentation at our clinic in Böblingen near Stuttgart, particularly due to its viral spread. What is BREAST IMPLANT ILLNESS anyway? First of all, breast implant illness is not recognized as a formal medical diagnosis. What you need above all after your breast augmentation is patience: the breast will only develop over the next few weeks, especially if the implant is placed under the muscle. This only happens over time as the tissue relaxes. The shape then changes, for example, from rather “mountainous and pointed” to “anatomically teardrop-shaped”. Whether or not you need to wear a compression strap for shaping depends on your choice of implant: If you have chosen implants with a polyurethane surface, there is no need to wear the strap. For implants with a smooth or textured surface, on the other hand, a compression strap may be advisable, depending on the shape. These are only needed for a few days until the swelling has significantly subsided. In order not to jeopardize success, rules of conduct must be observed. Scarring

Scarring is a completely natural process following an injury to the skin as a result of an accident or an operation. Wound closure begins with self-dissolving deep sutures, which take the tension out of the wound. Superficial skin sutures then bring the edges of the wound close together. Minimal differences in height between the wound edges are evened out using special plaster strips, such as Steri-Strip™. This leaves only a very fine gap after the procedure, which becomes a fine scar.
